Program Services



Las Trampas is committed to helping each person succeed.

Our programs emphasize a lifelong educational process that includes self-advocacy skills, evaluating risks and consequences, learning to manage strong emotions, communicating clearly, and the practice of these skills in real life situations.

Licensed Group Homes For Adults
Las Trampas currently has four group homes located in Pleasant Hill and Walnut Creek. All homes are licensed as Level 4 and staffed with around the clock support for adults with moderate to severe developmental disabilities.

Key to our residential services is staff support so each person can live in a neighborhood setting. At home, staff support cooking, shopping, housework, and personal care. Staff also provide transportation to recreational and social activities, shopping, and medical appointments. Each person can then meet the challenges of daily living while enjoying a sense of community and belonging.

Supported Living Services
As one of the original pilot programs through federal funding, CSLA (Community Supported Living Arrangements), Las Trampas has provided SLS since 1990 to residents in apartments and homes in Concord, Lafayette, and Pleasant Hill. Las Trampas uses a person-centered planning process to help consumers to develop their Individual Service Plan. It includes the consumers’ values and dreams about where they would like to live, their activities and plans for the future. Facilitators help to develop the circle of support of family, friends and neighbors with will guide the consumer and strengthen his/her opportunities for success.


The staff are committed to assisting people with disabilities with activities of daily living, budgeting, locating resources, staying healthy and achieving their goals.


Independent Living Services
Independent Living Services are designed for people who are able to live in the community with minimal staff supervision. Skills coached may include money management, safety, cooking, self-advocacy, household management, health and hygiene, housing issues, and recreation/leisure, depending on the assessment. ILS services are scheduled around the needs of the consumer. Using a relationship-based approach to Independent Living coaching, we facilitate the connection between the person and the community through the utilization of community resources and the person’s unique abilities and talents. Independent Living Staff perform an assessment of each individual, and use the assessment to guide the service plan. An individualized approach is used in all services and training. Independent living skills are taught in the consumer’s natural environments, both home and community.

Nursing Consultation
Consultation is provided to all residents of the group homes and nurses are available for individual consultation regarding medical issues. Services also include health monitoring, nursing care and medical case management.

DAY PROGRAMS



Adult Development Program
Programs provide daily activity for people requiring a high degree of support. The adult development program can accommodate people in a safe and nurturing environment and provides staffing ratios of 1:3, 1:2 and even 1:1, determined by individual need.

Las Trampas staff are trained and have extensive experience with people who need hands on assistance with every aspect of personal care and daily living. Las Trampas provides people with a wide variety of on-site and community activities developing and maintaining communications, social, intellectual and physical skills while insuring personal health and safety.

Adult Vocational Program
Working in small groups, with a staffing ratio of 1:6, consumers are earning paychecks in the community and on campus. Some of the employment opportunities include:

Las Trampas Janitorial
Las Trampas Consumers provide experienced and affordable services for local business and community facilities.

Las Trampas Shreds
Efficient and confidential document shredding services are offered to small businesses in our community.

Las Trampas Mails
Consumers perform bulk mailing, sorting, labeling and similar jobs. They are able to complete projects quickly and effectively.

ACTIVITIES ON CAMPUS



ACCESS ART Studio:
Fine Art instruction and support for artists with disabilities.

The ACCESS ART program is accessible to all consumers no matter what their level of support need. The program is staffed using the services of a certified Art Therapist and Community Student Fellows from the California College of the Arts, Center for Art and Public Life, working in concert with LAS TRAMPAS direct cares staff. Art Studio activities will be available daily.

Nutrition, food preparation and kitchen skills:
LAS TRAMPAS has rehabilitated the existing onsite kitchen to provide experiential activities and learning opportunities for consumers. A regular monthly activity, “CHEF’S TABLE” is conducted by a volunteer Chef.

Snoezelen Room:
A light, sound and tactile sensory environment available to all consumers.

Computer Room:
Six computers and inter-active software is available for interested consumers to learn and practice basic skills.

Physical Training Room:
Fitness room includes: Treadmill, Stationary Bikes exercise mats and equipment available to all consumers with supervision.

Music and Reading Lounge:
Consumers select and listen to the music of their choice. They also enjoy reading and social interaction.
